"Report a bug" page for non-Bugs-using project needs beautifying

Bug Description

The "Report a new bug" page for a project that doesn't use Launchpad for bugtracking, such as <https://launchpad.net/gst/+filebug>, has a few cosmetic problems:
* a warning alert, when you're not in any danger
* use of the obsolete term "Malone"
* use of a title= attribute in a question that's only nine words long already
* unnecessary (i) icon
* link to the package's Overview page, which would be better going to the package's Bugs page
* a yellow box with a stark black border and an obsolete rocket icon.

It would save time to fix this bug at the same time as bug 174446.
