UPDATE: This bug appears to be caused by a regression in the i915 driver. See the explanation and patch from Takashi Iwai here: https://lkml.org/lkml/2011/10/14/128 . The problem is fixed for me using the 3.0.4 kernel with the patch applied. See bug report and further discussion here: https://bugs.freedesktop.org/show_bug.cgi?id=41926 .
