Using pavucontrol, I am able to adjust the application level balance but not the output device balance.
Playing with the left and right sliders on the output devices tab, the behavior is erratic -- the sliders appear to want to stay in sync always (even when the left and right sliders are 'unlocked'), however, at random it is possible to make one lag with the other I was able to get to about 50% and 100% on the sliders once.
